History
In 2006 and 2007 ACOHS focused on the construction of a 2,160 square foot, 4-chair facility where clinical dental services are delivered and where community outreach services are based. The 9th Street Dental Center opened for business August 30, 2007. ACOHS is committed to providing friendly service to lower income community members who are not receiving regular dental care, serving both children and adults. Through a federal grant arranged by former Congressman Ralph Regula, the Center is equipped with the latest digital x-ray technology. In 2010, 2495 people received dental care at the Center.
Staff
Jonathan P. Roscoe, DMD, serves as the fulltime dentist at the 9th Street Dental Center. He is a native of Ashland and a graduate of Hillsdale high school. He completed his undergraduate work and his doctor of dental medicine degree at Case Western Reserve University. He is a Fellow of the International Congress of Implantologists and a member of the Ohio Dental Association and the American Dental Association. He has worked in dental practices in the Cleveland area.
Dr. Roscoe joins Dr. Ronald L. Freeman, Dr. Byron T. Leidtke and Dr. David M. Steury, all of whom provide services at the Center part-time. Support staff include dental hygienists, dental assistants, expanded function dental auxiliary and a director.
Services
Diagnostic, preventative, restorative, replacement and emergency services are available. This includes such services as exams, cleanings, sealants, fillings, extractions, root canals, crowns, bridges, dentures, implants and bone grafts. New patients are welcome.
Hours
The Center is open Monday through Friday from 8:30 am to 4:30 pm and Thursday evenings until 8 pm. To schedule an appointment call: (419) 289-1430 Walk-ins are accepted Wednesday mornings at 8 am.
Missed appointments waste valuable time and prevent others from receiving needed care. If you need to cancel an appointment, please give 24 hours notice so that we may offer the time to another patient. If you miss an appointment without notifying us, you may walk-in Wednesdays at 8 am or call any day you are available and if there is an opening, it will be offered. If you are more than 15 minutes late for an appointment, we may not be able to see you. If you miss three appointments, we will not be able to continue serving you.
Financial Policies
Medicaid, Caresource, Buckeye, Unison, Well Care, Molina and Dental Insurance are accepted. The 9th Street Dental Center is an in-network provider for Delta Dental and MetLife Dental. Current insurance or Medicaid cards must be presented at each appointment. A grant from Ohio Department of Health allows discounts to be given based on family income for uninsured patients in 2011. Income verification for the family must be presented to receive a discount. No one will be denied access to services due to inability to pay.
Payment is due at the time of service. Any other arrangements must be made with the receptionist prior to seeing the dentist. Balances must be paid before additional services are provided. Not all services are covered by all payment plans. Please ask the front office staff about which services are covered by your payment plan. Persons responsible for accounts are responsible for balances regardless of insurance coverage. Cash, credit cards, and checks with a valid driver’s license are accepted.
Registration Form May be printed, completed and brought to appointment.
